Submit a Student Code of Conduct Violation

This form is to be used to report a potential violation to the Student Code of Conduct. This form can be completed by anyone in the campus community. Violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, cheating, plagiarism or falsifying official records. This Reporting Form is NOT a 911 or Emergency Service. Reports submitted through this service will not receive an immediate response.
